An important part of medical treatment is obtaining information and keeping accurate records. We know it may seem like a hassle to fill out paperwork before you come to the clinic, but it really is crucial to your treatment for Dr. Hutchison to get a complete picture of what’s going on with you and your health. So please think of paperwork as the first step toward creating your family.

It’s useful to have the forms completed before you come for your appointment, so you can have your full hour with Dr. Hutchison on that day. Additionally, by sending the forms ahead of time, Dr. Hutchison is able to “do his homework” by reviewing your forms before he sees you.
